APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract amendment with Homebase, to increase the payment limit by $473,009 to an amount not to exceed $1,419,027, and to extend the term through June 30, 2027, to provide additional consultation and technical assistance services regarding continuum of care planning and resource development for the Health Services Department.
