APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract amendment with Contra Costa Interfaith Transitional Housing, Inc. (dba Hope Solutions), to increase the payment limit by $439,624 to an amount not to exceed $584,624, and to extend the term through June 30, 2027, to add rapid rehousing services in addition to homelessness prevention services for individuals who are at risk of or currently experiencing homelessness and referred by the Employment and Human Services Department‘s Housing Disability Advocacy Program. (100% State)
