APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract amendment with Herculean Babies Pediatrics, to include a pay for performance incentive to continue to provide pediatric primary care physician services for Contra Costa Health Plan members and County recipients with no change in the payment limit of $750,000 or term ending February 28, 2027. (100% Contra Costa Health Plan Enterprise Fund II)
