APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract amendment with San Ramon Urgent Care & Clinic, to include a pay for performance incentive to continue providing primary care physician services for Contra Costa Health Plan members and County recipients with no change in the payment limit of $1,500,000 or term ending December 31, 2027. (100% Contra Costa Health Plan Enterprise Fund II)
