APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract with Lime Ridge Healthcare LLC (dba Concord Post Acute), in an amount not to exceed $24,000,000 to provide skilled nursing facility and sub-acute facility services for Contra Costa Health Plan members and County recipients for the period July 1, 2026 through June 30, 2029.
