APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract amendment with Lyon-Martin Community Health Services, to increase the reimbursement rate for Medi-Cal Members to comply with the Department of Health Care Services rate parity requirement for Federally Qualified Health Centers and to continue providing primary care physician, medical specialist, and non-physician services to Contra Costa Health Plan members and County recipients with no change in the payment limit of $800,000 or term ending November 30, 2026. (100% Contra Costa Health Plan Enterprise Fund II)
