APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract amendment with Contra Costa Interfaith Transitional Housing, Inc. (dba Hope Solutions), to increase the payment limit by $2,332,000 to an amount not to exceed $6,749,156 to provide additional rapid rehousing and homeless prevention services for individuals who are homeless or at risk of becoming homeless with no change in the term ending June 30, 2027.
