APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract amendment with Catholic Charities of the Diocese of Oakland (dba Catholic Charities of the East Bay), to increase the payment limit by $778,214 to a new payment limit of $1,372,758 and extend the term through June 30, 2027 for additional homeless prevention services for individuals and families who are homeless or at risk of becoming homeless in Contra Costa County. (57% County General Funds Contingency Reserve; 36% Measure X; 7% Homeless Housing, Assistance and Prevention funds)
