APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract amendment with Bay Area Community Services, Inc., to increase the payment limit by $778,214 to a new payment limit of $1,741,820 and extend the term through September 30, 2027 for additional homeless prevention services for individuals and families who are homeless or at risk of becoming homeless in Contra Costa County. (45% County General Fund Contingency Reserve; 41% Measure X; 14% Housing and Homelessness Incentive and Prevention funds)
